Surgical Management of Prolonged Air Leak in Patients with Underlying Emphysema

چکیده
Prolonged air leak is one of the most common post-operative complications encountered after thoracic surgical operations involving mobilization or resection of lung parenchyma. Air leak typically manifests as persistent bubbling in a chest tube drainage system, but may also present with increasing subcutaneous emphysema or pneumothorax in a post-operative patient. No universal consensus exist as to the exact duration of air leak which constitutes a prolonged air leak, but it is generally regarded to exist if it is present for more than 5 days(14) or 7 days(2, 5-7) after initial surgery. It is an important complication that results in increased length of stay(8-15) and has been associated with other post-operative complications such as pneumonia(12, 14, 16), empyema(9, 10, 16) and ICU re-admission(12).

Asymptomatic expectoration of surgical staples complicating lung volume reduction surgery.
Lung volume reduction surgery (LVRS) has recently been introduced as a palliative treatment for patients with severe emphysema. The most common postoperative complication is persistent air leak requiring prolonged tube thoracostomy. We describe a unique case of a patient with severe emphysema who underwent LVRS and presented, about a year later, with the repeated expectoration of surgical staples.

OBJECTIVE Prolonged air leak occurs frequently after lung volume reduction surgery (LVRS) and can negatively affect both morbidity and hospital stay. We hypothesised that awake nonresectional LVRS could reduce the duration of air leak in emphysema patients. METHODS This analysis included 66 patients undergoing awake, unilateral plication of the most emphysematous lung regions under sole epidu...

PURPOSE Surgical operation for bullous emphysema is indicated for patients with symptoms related to the compression of giant bullae (usually >30% of hemithorax) or other related complications, such as infection, rupture, or bleeding. Video-assisted thoracoscopic surgery (VATS) has been widely applied in the diagnosis and treatment of patients with intrathoracic diseases, including bullous emphy...
متن کاملPneumopericardium after pneumonectomy and lobectomy.
Pneumopericardium is a rare condition, most frequently reported in connection with prolonged artificial ventilation in infants with hyaline membrane disease. No reports of pneumopericardium after pulmonary surgery have been published. Two cases of pneumopericardium are reported, one of tension pneumopericardium after pneumonectomy and artificial ventilation and one that followed radical lobecto...
متن کاملRadiotherapy: a novel treatment for pneumothorax.
Pneumothorax is a relatively common condition that is usually managed either conservatively, by chest tube drainage or, if a refractory air leak persists, then with cardiothoracic intervention. However, there is a small group of patients with a persistent air leak in whom surgical intervention is felt to be inappropriate. This study looks at a novel management strategy in a patient presenting w...
